
Home Medication Review
A home medication review is an assessment carried out between a pharmacist and GP, which can be claimed through Medicare on a yearly basis. ADMS doctors work with accredited pharmacists to oversee this process. Once the GP has made a referral to their pharmacist of choice, the pharmacist will arrange to meet with you and your caregivers at home.
The pharmacist will assess
- Safety of medications in use
- Education regarding appropriate administration
- Guidelines for medicines that require monitoring within a therapeutic range
The pharmacist and GP will discuss your medication and make changes if appropriate.
