Home Medicines Review

A Home Medicines Review is a service that can be organised by your GP. This will allow an expert pharmacist to look over your medications whilst in the comfort of your home.

Many people with a disability are on a complex list of medications. It is important that medications are regularly reviewed to see if they are still needed.  A Home Medicines Review allows both your GP and pharmacist to work together to come up with a medication plan for you.


The team at Adelaide Disability Medical Services can recommend pharmacists who have experience working with people who have a disability. If you would prefer to use your own pharmacist, please let your GP know.

Home medicines review eligibility

Am I eligible for a Home Medicines Review?

You may be eligible for a Home Medicines Review if:

  • You take at least 5 medications
  • You have recently changed medication
  • You are not responding to your medication as expected
  • You have at least 12 doses of medication each day
  • You have recently been discharged from hospital
  • You are having difficulty managing your medications
  • You are having side effects from your medications

What happens during the Home Medicines Review?

The pharmacist will contact you to organise a time to come to your house. You may have a support person with you for the appointment. The pharmacist will look at:

  • all medications (prescription and non-prescription) that you are taking
  • how, when and why they are taken
  • how the medications are stored within your home
  • how well you/your carers understand the medications and their effect
  • the use of any devices such as inhalers for administering medicines

Is there a cost for the Home Medicines Review?

There is no gap for a Home Medicines Review provided you have not had one within the last twelve months.

What happens after the pharmacist has completed the Home Medicines Review?

After the pharmacist has looked over your medication, they will send a report back to your GP. In this report, they will make any suggestions for possible changes to your medication. You will then need to make an appointment with your GP to discuss these changes further and come up with a medication plan for the future. This may involve reducing the dose of medications, stopping medications that are no longer needed or changing to different medications.
