It can be challenging to remember what to bring to your Doctor’s appointment.
We have prepared this checklist as a reminder for you, depending on the reason for the appointment.
✔ Am I able to consent to the appointment myself? If not, have I got consent from my Guardian/Parent?
✔ Have I brought my current medication chart(s)?
✔ Have my Medicare card or Concession/Pension card expiry numbers changed recently? If so, then I will need to bring these in.
Depending on the reason for my appointment, I may also need to bring:
✔ My communication folder which documents the events that have happened in the past
✔ My bowel chart
✔ My blood sugar chart
✔ My seizure chart
✔ Injection medication that is to be given today by my Doctor
✔ A blank medication chart so that my medications can be re-written
✔ My most recent hospital discharge summary, if the purpose of my appointment is to see how I am doing after being in hospital.
✔ Other charts that may help my Doctor to care for me during my appointment
